CHRISP STREET HEALTH CENTRE PATIENT CHARTER
Our commitment to you...
We are dedicated to providing a high quality service that meets your needs.
We will treat patients courteously.
We will involve you in decisions about your care.
You have the right to confidentiality at all times.
We offer you a personal family doctor for you to see for your long term care.
We aim to provide a sufficient number of appointments that will enable access to a GP within two working days, as well appointments which may booked up to 3 months in advance. We will also offer a range of appointments throughout the working day.
If you need a same day appointment, we will usually offer you a telephone appointment with a nurse or GP. You will be made a same day appointment if the nurse or GP considers one is needed.
Our receptionists may ask for information about your problem to identify the best health professional to help.
We will strive to run our surgeries to time. Where there is a long delay, we will inform you.
Patients with urgent medical conditionswill be given priority and will be seen assoon as possible by a doctor, even when this may cause delay to booked appointments.
When you join the practice you will be offered an appointment with a healthcare assistant, assistant practitioner, practice nurse or a doctor.
We will offer advice and seek to inform you of steps you can take to promote good health and to avoid illness.
We will publicise our services and events by means of our email list, text, website, TV, notice boards and leaflets.
Patients may choose whether or not totake part in research or student training.
Any suggestions to improve serviceswill be considered.
All complaints will be dealt with confidentiality and sensitively. A manager is usually on hand to discuss your concerns.
What we ask in return...
We ask that you are polite and courteous to our staff at all times. Patients who behave in an unacceptable manner may be removed from our list.
Please attend appointments on time or give the Practice at least one hour’s notice that you wish to cancel in order that your appointment is not wasted.
Remember that an appointment is for one person only - where another member of the family needs to be seen or discussed, another appointment should be made.
If you need to be see the doctor about a number of problems, or have language difficulties, you should request a “double appointment”
Patients should only request an urgentappointment if it is truly urgent.
Let us know in advance if you need a chaperone or translator.
Request a Home Visit before 11.00 am whenever possible. We ask all patients to make every effort to consult at the surgery where they can to make the best use of nurse and doctor time.
